How to Apply (Form Kaise Bhare)

1Visit your nearest Anganwadi Centre (AWC) or approved Govt. health facility within 150 days of your Last Menstrual Period (LMP).
2Ask for and fill out Form 1-A to register for the scheme. Provide your Aadhaar and bank details to claim the 1st installment.
3After completing 6 months of pregnancy and at least one health checkup (ANC), submit Form 1-B for the 2nd installment.
4After your child is born, register their birth, get them their first vaccinations, and submit Form 1-C for the final 3rd installment.
